What is the protocol when a sub gets injured on an assignment?

An injury may occur while a sub is working at an assignment. Please follow the steps below to ensure that our subs are getting the assistance they need.

STS Injury Protocol

**If it's a Medical Emergency, Call an Ambulance Immediately!**

1. If there is a school nurse, have the injured party be seen by them or another school medical staff member (if a medical staff member is not available, they can be seen by a member of the front office)
2. If the school nurse is unable to resolve the injury, setup a visit for them at Concentra Health

3. Complete the Concentra form below and send it to the Concentra location that the injured party is going to for the injury:

4. Obtain the incident or accident report from the school if available. Otherwise, use the form below:
